I, the undersigned, as the parent or legal guardian of the child named below, hereby grant permission for my child to participate in the Chatham Beach Volleyball Club’s competitive beach training program for the 2025 season. I understand that participation in beach volleyball involves inherent risks, including but not limited to falls, collisions, and other physical injuries that may occur during training, league play, or tournaments.
By signing this document, I acknowledge and agree to release, discharge, and hold harmless Chatham Beach Volleyball Club, its owners, directors, coaches, staff, and affiliates from any and all liability, claims, demands, or causes of action related to any injury, loss, or damage that may occur while on the facility premises or while participating in any club-sponsored activities.
I further understand that it is my responsibility to ensure my child is in appropriate physical condition to participate in athletic activities and that I assume all risks associated with their involvement. I confirm that I have adequate medical insurance coverage for my child and agree that in the event of an emergency, Chatham Beach Volleyball Club staff may seek medical attention for my child if necessary.
